Add a Survey
When you click the Add button on the survey child form, the Add-Accreditation Survey Area page opens. In netFORUM, surveys are also called survey areas. A Survey Area is an area of focus that will be examined to determine whether an institution is worthy of Accreditation. Examples of survey areas include curriculum, campus, staff, etc.
To Add a Survey Area, use the following steps:
- Go to .
- Click the Add button. The Add-Accreditation Survey Area page opens.
- Fill in the fields in the Survey – Add section as described in the table below:
Field Required Description accreditation Required Populates automatically with the accreditation program description. organization Optional Populates automatically with the name of the organization seeking accreditation. survey name Required Enter a friendly name for the survey. survey type Required Select a survey type from the drop-down list. survey team Optional Enter the first few letters of the team name to see a list of matches. Or use the look up icon. survey scope Optional Select a survey scope from the drop-down list. number of days Optional Once you select a survey scope, the number of days for this scope automatically appears. number of surveyors Optional Once you select a survey scope, the number of surveyors for this scope automatically appears. - In the Survey Fee(s) Section, select the check box beside each fee that applies.
- Click the total button. The total cost for all the fees updates.
- Select a confirmation method by clicking one or both of the following check boxes:
- send fax confirmation
- send e-mail confirmation
- Fill in the Invoice Section as described in the table below:

Depending on what type you select, you will see some or all of the fields below. po number Optional Enter a po number. Enter up to 50 characters, numbers or letters. payment amount Required Enter the amount of the payment received. payment method Required Select a payment method from the drop-down list. Depending on what payment method you select, you see some of the fields in the table in step 3. - Fill in the Payment Section as described in the table below. Depending on the payment method you choose, you see some of the following fields but never all of them.

TeleCheck cc/check zipcode Optional Check the existing zipcode. TeleCheck drivers license number Optional Enter the check holder's drivers license number. TeleCheck social security number Optional Enter the check holder's social security number. credit / TeleCheck cc/check email Optional Check the existing email address. - Click the Save button to add the survey area and return to the Accreditation Area Profile. Click the Cancel button to return to the Accreditation Area Profile without saving your changes.
