Editing Accreditation Survey Area Profile
The Edit button allows you to add and edit all of the accreditation survey area information on the Accreditation Survey Area Profile page.
Using the Edit Button
- Go to .
- Click the Edit-Accreditation Survey Area page opens.
- The Edit-Accreditation Survey Area page is divided into the following five sections:
- Survey Information
- Committee Dates
- Survey Scope & Dates
- Application Dates
- Confirmation & Packet Dates
- Fill in the fields for populated accreditation and organization fields in the Survey Information section as described in the table below:
Field Required Description survey name Optional Enter the name of the survey. survey type Optional Select the survey type from the drop-down list. date entered Optional Select or enter the date you added the survey. begin date Optional Select or enter the date on which the survey begins. end date Optional Select or enter the date on which the survey ends. - Fill in the fields for the Committee Dates section as described in the table below:
Fields Required Description mailed to committee on Optional Select or enter the date the survey materials were mailed to the committee. return to committee on Optional Select or enter the date that the survey was returned to the committee from the organization being surveyed. conference call on Optional Select or enter the date for a conference call about the survey. decision letter sent on Optional Select or enter the date that the committee sent the decision letter to the organization. draft sent on Optional Select or enter the date the draft survey was sent to the organization being surveyed. certificate sent on Optional Select or enter the date that the accreditor sent the certificate to the accredited organization. - Fill in the fields for the Survey Scope & Dates section as described in the table below:
- Fill in the fields for the Application Dates section as described in the table below:
Fields Required Description received on Optional Select or enter the date on which the accreditor received the application. fee paid on Optional Select or enter the date on which the accreditee paid the application fee. completed on Optional Select or enter the date on which the accreditee completed the application. - Fill in the fields for the Confirmation & Packet Dates section as described in the table below:
Fields Required Description team confirmed on Optional Select or enter the date on which the team confirmed that they can do the survey. packet prepared on Optional Select or enter the date on which the survey team prepared the survey packet. confirm letter sent on Optional Select or enter the date on which the accreditor sent a confirmation letter to the accreditee. packet sent on Optional Select or enter the date on which the accreditor sent the packet. - Click the Save button to save your changes and return to the Accreditation Survey Area Profile.
Click the Delete button to delete the accreditation survey area.
Click the Cancel button to return to the Accreditation Survey Area Profile without saving your changes.
