Parameter Name	Required	Description
Survey	Required	Choose the specified survey you want to run this report on from the drop-down list.
Survey Group(s)	Optional	Choose the specified survey group(s) you want to run this report on from the list box.
Group By	Required	Choose the specified group by you want to run this report on from the drop-down list.

Report Fields

This report can produce output in a browser window (HTML) or as a PDF containing the following information:

Column Name	Description
Customer	Lists the customer name.
Survey Question	Lists the survey question.
Response Date	Lists the response date.
Response	Lists the survey response.
