Member Type	Required	Choose the specified member type you want to run this report on from the drop-down list.
Join Date From	Optional	Choose the specified join date from you want to run this report on from the date text box.
Join Date To	Optional	Choose the specified join date to you want to run this report on from the date text box.
Expire Date From	Optional	Choose the specified expire date from you want to run this report on from the date text box.
Expire Date To	Optional	Choose the specified expire date to you want to run this report on from the date text box.
