Accreditation Survey Area Profile
In NetForum, a survey is also a survey area. The Accreditation Survey Area Profile allows you to view and modify information about one survey that is associated with an Accreditation Program and an Organization.
Accreditation Area Profile Information
The Accreditation Survey Area Profile displays information in the following sections:
- Survey Information
- Committee Dates
- Survey Scope

Survey Information
| Field | Description |
|---|---|
| accreditation | Displays the name of the accreditation program. |
| organization | Displays the name of the organization. |
| survey id | Displays the NetForum-generated survey number. |
| survey name | Displays the descriptive name of the survey |
| survey type | Displays the user-selected type of survey. |
| date entered | Displays the user-selected date that the survey was entered. |
| pre-survey materials received on | Displays the date that the organization reported they received the pre-survey materials. |
| begin date |
Displays the date on which the survey begins. |
| survey fee paid on | Displays the date that the organization paid the survey fee. |
| end date | Displays the date on which the survey ends. |
Committee Dates

| mailed to committee on | Displays the date the survey materials were mailed to the committee. |
| conference call on | Displays the date for a conference call about the survey. |
| draft sent on | Displays the date the draft survey was sent to the organization being surveyed. |
| return to committee on | Displays the date that the survey was returned to the committee from the organization being surveyed. |
| decision letter sent on | Displays the date that the committee sent the decision letter to the organization. |
| certificate sent on | Displays the date that the accreditor sent the certificate to the accredited organization. |
Survey Scope

| number of days | Displays the number of days a survey of this scope will take. |
| number of surveyors | Displays the number of surveyors a survey of this scope will take. |
| survey team | Displays that name of the survey team assigned to this survey. |
Application Dates
| received on | Displays the date on which the accreditor received the application. |
| completed on | Displays the date on which the accreditee completed the application. |
| fee paid on | Displays the date on which the accreditee paid the application fee. |
Confirmation Dates
| team confirmed on | Displays the date on which the team confirmed that they can do the survey. |
| confirm letter sent on | Displays the date on which the accreditor sent a confirmation letter to the accreditee. |
| app. completed on | Displays the date on which the application was completed. |
Packet Dates
| packet prepared on | Displays the date on which the survey team prepared the survey packet. |
| packet sent on | Displays the date on which the accreditor sent the packet. |
Accreditation Survey Area Profile Actions
You can perform the following actions from the Accreditation Area Profile:
- Edit the Profile
- Add an Affiliated Program (add branches)
- Add a Survey
Accreditation Survey Area Child Forms
The child forms section of the Accreditation Survey Area Profile is divided into three tabs. Each tab contains a child form with information related to the accreditation survey area.
