Participating in a COI Disclosure

Once a request has been made, it will be visible on eWeb for the individual to whom the request was made. Individuals will access the questionnaire from their My Account page and submit their answers. Once a questionnaire has been submitted it will be available for review.

Viewing and Submitting a Disclosure on eWeb

Once a request has been submitted, the disclosure, in the form of a questionnaire, will be available on eWeb.

To view and submit a disclosure on eWeb, complete the following steps:

  1. Log into the eWeb site for your association.
  2. Hover your mouse over the Extra Features menu item located along the top navigation bar.
  3. Click the COI Disclosures link.

    This opens the Conflict of Interest - My Disclosures page. This page lists all Outstanding Disclosures and Submitted Disclosures.

  4. Click the linked name of the disclosure you wish to view or submit.

    This opens the COI Disclosure Response page.

  5. Answer the questions on the questionnaire.
  6. Click the Save button.

    Once you click the Save button, your answers will be recorded and you can no longer modify them. The questionnaire is also flagged as ready for review and is available to the reviewers for this disclosure.

    Once saved, the disclosure moves from the Outstanding Disclosures section of the Conflict of Interest - My Disclosures page to the Submitted Disclosures section.

    Note: You can open the disclosure in read-only mode by clicking the linked name of a disclosure in the Submitted Disclosures section. You can view, but change your answers.
