Report Parameters

Selected report parameter information displays below the title of the report.

Parameter Name	Required	Description
Survey Name	Optional	Choose the specified survey name you want to run this report on from the drop-down list.
Survey Date From	Optional	Choose the specified survey date from you want to run this report on from the date text box.
Survey Thru Date	Optional	Choose the specified survey thru date you want to run this report on from the date text box.

Report Fields

This report can produce output in a browser window (HTML) containing the following information:

Survey Description	Lists the survey description.
Survey Purpose	Lists the survey purpose.
From Date	Lists the survey from date.
Thru Date	Lists the survey thru date.
Point of Contact	Lists the survey point of contact.
