Committee Roster 3-Column Format Report

This report list committee participants per committee in a 3-column format.

Report Parameters

Parameter	Description
Affiliate	Select an Affiliate from the drop down list.
Committee Code-Name	Select a Committee from the drop down list. This list changes based on the selection made for Affiliate.
Include Certification Designation?	Click the box to include certification designation in the report.
Include Additional Contact Info?	Click the box to include additional contact information in the report.
As of Date	Enter a date, or select a date from the drop down calendar, for the report.

Report Fields

Field	Description
Report Header	Lists the Name and committee selected for the report.
Committee Member	Lists the name, position on the committee the member occupies, and contact information for the member.
