Individual Profile Child Forms

Individual profile child forms allow you to view and, in some cases, add additional information on an individual. Child forms are organized under tabs, and each tab contains several child forms related to the tab topic.

Click a tab to view child forms containing more information on a wide range of aspects of the individual.. Click the expand button next to a child form title to view its contents. Click the pop-out window icon to open the child form in a new window.

Membership tab child forms include:

  • Membership:  Shows information on the individual membership status and type, as well as expired membership information. This child form also indicates whether a membership is Primary for the individual.
  • Dues Orders:  Shows details on dues for memberships that have been purchased by the individual.
  • Recurring Payment:  If the individual has a recurring renewal membership, recurring payments appear in this child form.
  • Membership Log:  Displays the history of and changes to the individual's membership status.
  • Membership History from National: Displays the history of the individual's dependent memberships.
  • Membership Flow: Displays member benefits that flow down from a linked organization to the individual. In the example below, the organization set as the individual's primary organization has a "Diplomate" membership type that has flowed down to the individual.
 

Relations tab child forms include:

  • Linked Organization:  Lists organizations for which the individual is the primary contact.
  • Linked Individuals: Lists all individuals linked to the individual whose profile is being viewed.
  • Relationships to Organizations: Organizations in this child form have a relationship to the individual. Such a relationship might the individual's status as a CEO, board member, employee, or so on.  Select organizations to add to this list by clicking Add at the top of the child form. (For more information, see: Adding an Individual's Relationship to an Organization.)
  • Relationships to Other Individuals: Individuals in this child form have a relationship to the individual being viewed. Such a relationship might be a mentor, a parent, or a manager. Select individuals to add to this list by clicking Add at the top of the child form.
  • Relationships from Other Individuals: Lists relationships to other individuals linked to the individual whose profile is being viewed.  These relationships are made from the profile page of the other individual.
 

Purchases tab child forms include:

  • Merchandise Purchased: Lists merchandise purchased by the individual, as well as purchase information.
  • Miscellaneous Items Purchased: Lists items categorized as miscellaneous that have been purchased by the individual.  (For more information on miscellaneous products, see: Setting up Miscellaneous Products.)
  • Donations/Funds Purchased: Instances of donations and funds purchased by the individual are listed in this child form.
  • Merchandise History from National: Displays the merchandise history of the individual's dependent memberships.
  • Late Fees: Lists late fees for purchases that were not paid for by their due dates.
 

Subscriptions tab child forms include:

  • Current Subscriptions:  All current subscriptions that the individual has purchased are listed in this child form.
  • Purchased Subscriptions:  All non-current subscriptions that the individual has purchased, including canceled and expired subscriptions, are listed in this child form.
  • Subscription Change Log: The subscription log tracks details of subscription renewals, cancellations, and other changes.
  • Subscription History from National: If your netFORUM Pro database uses SYNC functionality to synchronize data from Enterprise to netFORUM Pro Pro, subscription history will appear in this child form.
 

Activities tab child forms include:

  • Event Registrations:  Events for which the individual has registered are listed in this child form.
  • Speaker:  Events for which the individual is a speaker are listed in this child form.
  • Committees:  If the individual is a member of any committees, they are listed here. The individual may be added to committees by clicking the Add button in this child form.
  • Subcommittees:  If the individual is a member of any sub-committees, they are listed in this child form.
  • Group Registrations:  If the individual has purchased a group registration to an event, a note will appear under this child form. (For more information on registering a group for an event, see Registering a Group Using the Event Registration Wizard.)
 

Certifications tab child forms include:

  • Designations:  This child form lists any designations earned from participation in a certification program. Example of designations could include acronyms such as CPA or RN.
  • Certification:  This child form lists all certifications that have been earned by the individual, including information on certification program, status, certification status, certification number, certification date, effective date and expiration date.
  • Education Credits: These are education credits that the individual has earned from participation in certification programs. Add to this list by clicking the Add button at the top of the child form.
 

The Awards tab child form is:

  • Awards:  The Awards child form lists award entry information, including award entry number, name, category, status, nominator, received date, average score, final score, fees paid, and whether or not the individual won the award.
 

Notes tab child forms include:

  • Notes:  This child form lists notes added via theAdd Notesaction button on the individual profile.
  • Documents:  Lists documents of interest that have been uploaded to the individual profile. To save a document in this list to your hard drive, right-click the File Description link and click Save Link As.  Upload new documents by clicking the Add button in the child form header.
 

Other tab child forms include:

  • Interests: Information on the individual's interests is listed in this child form. Add interests by clicking the Add button in the child form header.
  • Activity Codes:  This child form allows you to select from a drop-down menu of activity codes. Select more activity codes by clicking the Add button in the child form header. (For more information on activity codes and adding activity codes, see Setting Up the Activity Code Drop-down List.)
  • Historical Activity: The individual's historical activity can be recorded in this child form, including activity codes, description, cost, date, and reference information. Add to this child form by clicking the Add button in the child form header.
  • Contact Request:  The Contact Request child form lists contact requests made by the individual, either by calling in to staff or from the eWeb interface.  Record contact requests by clicking the Add button in the child form header.
  • Manual/Overpayment Credits:    This child form lists all overpayment credits made to the individual, as well as manual credits that have been created using the Actions drop-down Add Manual Credit feature. Credits made via the cancellation process do not appear under this child form.
 

Correspondence child forms include:

  • Communications:  The Communications child form lists all communications with the individual and lists date, an activity description, activity code, and method of communication.
  • Mailing List:  When the individual is added to a mailing list, it will appear in this child form.
  • Opt Out List:  The Opt Out List child form lists the types of e-mail correspondence that the individual has opted out of.
  • Contact Request:   The Contact Request child form has been moved from the Other child form tab, and now appears under the Correspondence. The Contact Request child form lists contact requests made by the individual, either by calling in to staff or from the eWeb interface.  Record contact requests by clicking the Add button in the child form header.
 

Merged Info child forms include:

  • Addresses
  • Phone Numbers
  • Fax Numbers
  • E-mail Addresses

Child forms under the Merged Info tab display address and contact data that has been merged from a duplicate record. Data appears under the Merged Info tab child forms only if a duplicate organization record was merged with the original organization record using the Merge and Purge tool.

 

Log child forms include:

  • Address Change Log: Changes to the individual's address are logged here.
  • Change Log:  This child form records changes to the individual's data, including phone, fax, e-mail, relationship from individual to organization, and relationship from organization to individual. Changes that are in the Change Log that have a date more than 6 months old will be cleared out by a nightly process.
